HEALTH INSURANCE

You deserve to have the peace of mind of knowing you and your family are covered if you get sick or injured. Medicaid is low-cost or free health insurance that can help you get the care you need and protects you from large medical bills. It covers doctor visits, hospital care, prescriptions, mental health care, and more. If you are not eligible for Medicaid you can apply for and enroll in Marketplace health coverage.

Medicaid and Marketplace applications may request different information from state to state, so make sure you have everything you need to enroll in a health insurance plan. Here's a general checklist:

What do you need to get covered?

• Marketplace account username and password

• Social Security sard or Permanent Resident Card

• Proof of income, such as a W-2 form, 1099 form, or pay stub

• List of your current doctors and medicines
